Acknowledgement, indemnity and POPIA consent

I, the undersigned, acknowledge that the activities and exercises offered will cater to various skill and ability levels. I will use my personal judgment to ensure I do not unduly strain my body or cause personal injury. By signing this form, I agree to indemnify and hold harmless Serene View Pty (Ltd) T/A Yoga Connection (hereinafter referred to as Yoga Connection), its Directors, Staff, Instructors and Teachers for any injury sustained in any manner whatsoever. I further acknowledge that I enter the property and premises at 243 Brooks St, Brooklyn, at my own risk, and do not hold Yoga Connection, its Directors, or the property owners responsible for any loss or damage to myself or my property that may occur in any way. I acknowledge that the results/success of the Yoga classes depend on many factors and neither Yoga Connection nor the Instructor can therefore guarantee results/success. I acknowledge that the Instructors are solely responsible and liable for their behaviour as independent Yoga Instructors. I hereby grant consent to Yoga Connection, its Directors, Instructors, Representatives, and the Administrative staff to access, collect and process my personal information and may use my personal information to communicate with me in person, via telephone, email, WhatsApp, and any other form of social, electronic, or written media. I accept that my personal information may be hosted on servers managed by a third-party service provider, which may be located outside of South Africa.
